Crucial Housing and Support Services
Foothill Family Shelter provides crucial housing and support services to homeless families in San Bernardino County and Los Angeles County. Our on-site services increase residents' capabilities to become self-sufficient. Services are just as vital as housing for the recovery and stability of homeless families.
Educational Scholarships & Healthcare Stipends
We provide educational scholarships and healthcare stipends. These services include case management; individual, group, couples and family counseling for children and adults; a literacy program/reading room -after school program K-12; parenting classes; a computer lab; resume/employment advocacy; legal advocacy; credit counseling and one-on-one professional support services. Services are available in English and Spanish. Participants in need of other languages will be referred to agencies that can meet their needs.
120 Day Program - Additional Time
Our 120-day program was created to provide additional time and assistance to families trying to break the cycle of homelessness. We recognized that families needed a solution and case plan, not just free shelter and food. In the 120-day program, each family stays in a furnished two-bedroom apartment with no charge for rent and utilities. Residents are required to save 50% of their income in a trust account and create to implement an employment/education case plan to ensure financial health, sustainability and independence.
Graduation from the Four-Month Program
Upon graduation from the four-month program, clients may access our continuum of care. Our continuum of care includes one-year low-cost housing and affordable permanent housing. All programs offer dignified, independent living coupled with supportive services -based on self-accountability - to ensure that clients transition from low-functioning dependency to sustainable, community contributors.
Helping Families in the West End
Foothill Family Shelter is one of two transitional housing and support programs in the West end of San Bernardino County. Our target population is families with four or less children and singles who are currently homeless who need a hand up to break the homeless cycle. We serve all of San Bernardino & a portion of Los Angeles County. Foothill Family Shelter's program is a nationally recognized program model, which is outcomes based and incentive driven.
Case Management
Once a client is accepted into our 120-day program, they are assigned to a Case Manager. Each family will receive a Psychological Evaluation and Depression survey, which are completed during the first 7 days in the program. Drug tests & alcohol testing are run on the adult family members and sent to a lab for analysis. Based on the assessment, the Case Management Team and residents agree on case plan goals and identify and develop a plan for reaching each goal. Within the family plan, each adult and child has an individual mental health plan. The Case Manager meets with residents weekly to document that services are delivered, to identify and resolve problems that may occur with accessing and utilization of services; to identify changes in residents' needs; and any additional services needed. Case notes and progress are maintained in the confidential case file. Monthly meetings are held with the Case Management Team and reassessments are conducted as needed. The Case Manger and Residents sign and date the initial case plan and subsequent modifications.
